The centripetal resorption of the edentulous maxillary results in a topography mechanically unfavorable to the proper functioning of implant-supported reconstructions. This bone re-arrangement often inverts the transverse interarch relationships.
The case described in this chapter requires a three-dimensional bone rearrangement associating a sinus graft with an apposition graft to change a Siebert's Class III into a class I.
The new anatomical configuration allows implants, placed in a second step, to function on an optimal axis matching the biomechanical requirements of the implant-supported prostheses.
The case described in this chapter is a class VI case according to the classification we have set.
